What is a Care Manager?

A care manager is a professional who specializes in assisting with care assessments and care coordination. Our care managers have a minimum of a Bachelorís degree or equivalent training in gerontology, social work, nursing, counseling, or human services.

Our care managers are:

     Experienced nurses and/or social workers, providing clients with objective needs assessments and service recommendations;

     Care advocates, addressing care concerns and acting on the clientís behalf;

     Community resource specialists, offering clients accurate information on service providers, housing options, agencies and programs;

     Liaisons between clients and their families and caregivers, providing peace of mind by ensuring that loved ones receive the highest quality of care.
